ABOUT
Industrial Accident Investigation Services, part of 'The Safety and Training Academy' operate both in the UK and internationally, its owner has investigated over 200 fatal and high profile industrial and occupational accidents for clients, litigation and prosecution purposes across many countries.We are experienced in undertaking comprehensive incident and accident investigation and the preparation of reports for both criminal and civil proceedings and also providing the service of specialist witnesses. Industrial accident reports range from slips, trips and falls, through manual handling incidents, to plant and machinery and construction hazards, systems of work analyses, examination of risk assessments and method statements, investigation of repetitive-type injuries, through to fatal and life changing injuries.
As investigators we dig deep into all investigations to find both the primary or underlying cause (root cause) of the accident and all contributory causes, what we do not do is assign blame.
Where the accident occurs advice on the review of Health & Safety Management system to meet the requirements of the Management of Health & Safety at Work Regulations 1999 is given.
Workplace Accident and Incident Investigation
Workplace accident investigations are designed to identify the root cause of workplace accidents to help prevent future accidents. Effective reporting should follow a standard format and may require the involvement of several people, depending on the severity of the accident/incident. The investigation team should be trained on how to conduct investigations, and complete and analyse accident investigation reports.
Reasons for investigating accidents
Accidents may be an indication that the safety program, workplace communication, training, supervision or work procedures are deficient. By finding the root cause, or multiple causes, of an accident, necessary improvements can be identified and made in the organisation. Investigations are not designed to blame an employee and it is essential that all workers understand this. Accidents typically have multiple root causes and only by investigation of the circumstances will these causes be identified for corrective action.
Examples of accidents that may need investigation include:

Injury, illness, or property damage —This includes when employees, contractors, and the general public are involved.
